A death certificate contains personal information about the decedent, the circumstances surrounding the death, and the officials who certified and/or pronounced the death. The death certificate  is used to generate various death-related reports and notices. A funeral director’s responsibility is to take all the information about the decedent and complete the death certificate. If the funeral director is not available, the town clerk can fill out the death certificate for a funeral home within the town clerk's jurisdiction.

Verification of Information while Filling out the Certificate

When entering the decedent's information online, a verification application is in place to verify certain information from the Social Security Administration (SSA). The information that is being verified is contained in the first tab of the online death certificate. It includes:

When the record is saved using the SAVE button, the Online Verification System (OVS) verifies the information using the back-end OVS (Online Verification Subsystem) and provides the information has been verified or some of the data has not passed the authorization check. Click OK to continue.

If during the filling out of the record, you make a change to one or more of the previously mentioned items in the record, OVS verifies the change, and comes back with a message indicating the verification status.

You can make up to five changes during the filling out of a certificate.

OVS will only verify the SSN five times for the same death record. After the fifth attempt, a message is displayed stating the  number of verifications for the SSN has exceeded the limit.

Prerequisite

You have logged into the NHVRIN application using your user ID and password.

Steps

To add a death record from a death certificate:

  1. Click the Death tab.

  2. Select Record from the Death menu bar.

  3. Select Add from the drop-down list. The death certificate form is displayed.
